Interview with Dr. Laungani on Blepharoplasty and Plastic Surgery

Dr. Alexis Laungani, a plastic surgeon passionate about anatomy and reconstruction, shares his fascinating journey—from his studies at the University of Liège to his specializations in aesthetic and pediatric surgery. In this interview, he discusses his career path, his approach to blepharoplasty, and the importance of providing each patient with a reassuring and personalized experience throughout their surgical journey.

Could you tell us about your professional journey and what led you to become a plastic surgeon?

(A.L) – I began my medical studies at the University of Liège in Belgium from 2001 to 2008. During this time, I had the honor of working under Professor Alain Carlier, a renowned hand surgeon and head of the human anatomy department. Collaborating with him on various projects deeply inspired me and led me to author a book that is still used by medical students to learn human anatomy.

During my studies, I had the opportunity to work alongside plastic surgeons in anatomy labs. These encounters opened my eyes to reconstructive techniques, especially through work on flaps. Gradually, my interest in plastic surgery grew. I completed several internships in the field and was captivated by what I discovered.

After passing the surgery exam, I was selected for my desired position in plastic surgery. My first residency primarily focused on aesthetic surgery, with an introduction to head, neck, and breast reconstruction. Subsequently, I decided to pursue further studies in the United States, continuing anatomical research on various reconstruction flaps. My second residency was highly enriching, allowing me to explore diverse aspects of plastic surgery.

What is blepharoplasty, and why do patients often choose this procedure?

(A.L) – Blepharoplasty is a surgical procedure designed to rejuvenate the eyes by addressing the tired appearance caused by excess skin or fat, often exacerbated by time and gravity. As we age, tissues can sag, sometimes accompanied by bone loss, which can obstruct vision and create fat pockets around the eyes, resulting in a fatigued look.

Before considering surgery, non-surgical options such as neuromodulators to lift the brows slightly or hyaluronic acid injections in the tear trough can be explored. However, these treatments have limitations, especially when excess skin is present.

Upper blepharoplasty involves removing a small crescent of skin from the upper eyelid, concealed in a natural crease. This procedure may also remove fat deposits, particularly in the nasal corner of the eyelid.

For the lower eyelid, more fat than skin is typically removed since there’s less skin to excise. The often-weakened lower eyelid can be repositioned to improve its tone and smooth the transition between the cheek and eyelid, reducing the visible puffiness that most patients find troubling.

Blepharoplasty is an effective solution for achieving a refreshed and rejuvenated appearance.

Can you describe the preoperative process for upper blepharoplasty and what patients can expect on the day of surgery?

(A.L) – The preoperative process always begins with a consultation where we discuss the goals of the surgery and address any questions. This is the time to review your medication list and medical history. We specifically look for any medications that could increase the risk of complications. Together, we’ll decide if any medications need to be paused before the procedure.

We’ll also go over pre- and post-operative recommendations, including temporarily stopping certain medications and avoiding smoking. Once everything is in place, we’ll schedule the surgery date.

Upper blepharoplasty is performed under local anesthesia, and fasting is not required. Before the procedure, we’ll review and sign the informed consent, discuss surgical details, potential complications, and alternatives. Photos will be taken, and preoperative markings will be made before numbing the upper eyelid. Once you’re comfortable, the procedure, which lasts about 30 minutes, will begin.

You can return home after the surgery, accompanied by a family member or friend. It’s recommended to have someone with you for daily tasks during the first 24-48 hours.

How do you ensure that every patient feels comfortable and confident throughout the process, from the initial consultation to recovery?

We strive to create a welcoming atmosphere, maintaining honest yet warm communication. I spend time listening to my patients during their first visit, ensuring they feel at ease as we discuss the surgical plan in detail. I explain the expected outcomes to set realistic expectations.

We support our patients throughout the process—answering questions, providing additional information, or scheduling multiple consultations before surgery to alleviate any concerns or doubts. On surgery day, we ensure they feel comfortable by providing a familiar and safe environment.

I make myself available for post-operative appointments, and our team is always ready to address questions during recovery. By offering detailed information, being accessible, and truly listening, we help patients feel genuinely comfortable. This turns their journey into a positive experience they’ll remember fondly.
